The Home Care Package Program is designed for people who require more complex, specialised, individual-coordinated support.
A Home Care Package is a flexible package of support that enables you to live independently in your own home for as long as you can. A Home Care Package enables you to choose the services you need to meet your goals, support you to have a meaningful and safe life and you will have control over your own budget. Services may include those provided by Community Home Support Program and can include other health-related services.

What do you need to do to access these services?

If you have not already been assessed by an ACAT for a Home Care Package, you (or your nominated representative) need to call My Aged Care on 1800 200 422.
The contact centre staff will ask questions to help them understand your needs and care arrangements to inform the next steps.
Once you have been assessed, let the assessor know you would like to receive services from Astley Care.
