Choosing Astley Care as your Provider
We want you to live your best life the way you choose. If you wish Astley Care to be your provider, the most important thing is that we take the time to get to know you.
One of our Care Coordinators will arrange a home visit with you and any other people you would like to be with you. We will work with you to identify what you see as important to you and develop a plan of support based on your requirements and what you would like to achieve.

What types of support can you receive with Astley Care?
Home Care Packages are provided in a ‘Consumer Directed Care Model’, which means you choose the types of care and services you receive using the funds provided within your budget.
What do you need from us to help you live your best life? Help us understand what you need, and we can support you with what you need.
Astley Care offers support such as domestic assistance, shopping, personal care, medication support, transport, respite, nursing services and home maintenance, or you can use your funding to purchase other services. These include physiotherapy, podiatry, attendance at Day Clubs or gutter cleaning services.
You may also want to use HCP funds to purchase equipment to improve your independence and quality of life. Your HCP Care Coordinator will help you navigate the jargon, explain how your budget works, help you access external services or purchase equipment, and provide a monthly statement showing how your funds have been spent.
